How to use your Ministry of Health (MOH) funding for purchases with Naturalwear

You will need to phone the MOH to find out how much funding you have available.
Dial 0800 855 066 and follow the prompts #3 and #5. Make sure to take down your balance and expiry date.

Option 1: For using only your MOH funding

  1. Download and print the MOH Claim Form.
  2. Fill out the sections highlighted in yellow.
  3. Send the completed forms to us via email or post, and include your funding details:
    • For Email – Scan and send the form to [email protected]
    • For Post – Mail the form to PO Box 24-341, Royal Oak, Auckland

Once we receive the completed paperwork, we will courier the goods to you.

Option 2: For reimbursement after an online purchase

  1. Use your credit card to make your purchase.
  2. Naturalwear will process your order within 2-3 business days unless there is a back order delay.
  3. Goods arrive at your nominated address.
  4. Download and print the MOH Claim Form.
  5. Fill out the sections highlighted in yellow.
  6. Send the completed forms to us via email or post, and include your funding details:
    • For Email – Scan and send the form to [email protected]
    • For Post – Mail the form to PO Box 24-341, Royal Oak, Auckland

Make sure to include

• Your order number
• Your bank account details for a refund

After verifying your details, Naturalwear will lodge the claim with MOH.
The reimbursement process typically takes about 4-5 weeks.
